1. Private Pay (via Personal Check / Cashier's Check / Zelle Payments to 'marigoldalh@gmail.com')
2. LTC: Long Term Care Private Insurance (Most Insurance Companies accepted)
3. Arizona Complete Health ALTCS (AHCCCS Medicaid): Starting 10/1/2024, we are contracted with them and can take residents who are planning on going to ALTCS in the near future (Medicaid Spend Down) or already are on ALTCS

1. COVID-19 negative test
2. TB test (No signs/symptoms of TB)
3. Prescriptions as ordered by the Doctor (including OTC drugs) / Part of 'Physician's Paperwork'
4. POA copy (when applicable)
5. Social Security/Pension/Benefits statement (when applicable)

      All documents can be faxed to 480-452-1525
